Updated ISBE Form 34-43 Now Available

The Illinois State Board of Education has posted a new version of ISBE Form 34-43 –Application for Room and Board Reimbursement of Emergency and Student-Specific Placement in a Non-Approved Residential Facility. Placing school districts should begin using this version of the form, dated April 2024, for all emergency and student-specific placements allowable under 105 ILCS 5/14-7.02(e). Previous versions of the form will not be accepted after April 15, 2024.
